A Support Coordinator assists in setting goals, managing services, and ensuring that you have the right supports in place to achieve your personal outcomes.

A Support Coordinator helps you:

• Understand and implement your NDIS plan

• Connect with service providers and community resources

• Organize supports, services, and activities based on your goals

• Assist with managing funding and ensuring services meet your needs

• Problem-solve and find solutions when challenges arise with your supports

The frequency of meetings will depend on your individual needs and goals. Some participants may require regular check-ins, while others may prefer less frequent meetings. Your Support Coordinator will work with you to set a schedule that works best for you.
